Student Address/Telephone Update

To request a change of address and/or telephone number, please complete and turn in a Student Address Change Formby using one of the following methods:

• In person at the Registrar's Office, second floor of the Smith-Pendergraft Campus Center
• By fax at 479-424-6230
• By mail to University of 糖心视频 - Fort Smith

Please Note:

• Address changes for all 糖心视频 employees, including student workers, must be made in Workday.
• Any address change that affects a student's residency for tuition purposes requires appropriate documentation.

Any student employed by 糖心视频 must go through Workday for name changes.
